Pass the National Board Dental Hygiene Examination (NBDHE). Format: 350 multiple-choice questions, computer-based. Passing score: 75.
Pass a clinical board examination. Indiana accepts: CDCA (Commission on Dental Competency Assessments), NERB (North East Regional Board), WREB (Western Regional Examining Board), CITA (Council of Interstate Testing Agencies), CRDTS (Central Regional Dental Testing Service), SRTA (Southern Regional Testing Agency).
Pass the Indiana dental jurisprudence exam. Indiana Laws and Rules Examination (Ethics, Professional Responsibility, and Indiana Statutes/Administrative Rules).

Local anesthesia requires a separate Dental Hygiene Local Anesthesia Permit ($25), approved educational course (minimum 15 hours didactic + 14 hours lab), CPR training, and direct supervision of a dentist.
