Pass the National Board Dental Hygiene Examination (NBDHE). Format: 350 multiple-choice questions, computer-based (administered at Pearson VUE). Passing score: 75.
Pass a clinical board examination. District of Columbia accepts: CDCA.
Pass the District of Columbia dental jurisprudence exam. DC Dental Hygienist Written Take-Home Law Examination.
Obtain current CPR/BLS certification, pass a criminal background check, Local anesthesia authorization (optional, requires Board approval), Nitrous oxide authorization (optional, requires Board approval).
Submit your application to the District of Columbia Board of Dentistry.

DC dental hygienists can obtain Board authorization for local anesthesia and nitrous oxide, but both require direct supervision of a dentist. As of recent board data, 84 hygienists hold local anesthesia and nitrous oxide authorization, and 2 hold nitrous oxide only authorization. The Board limits the ratio of supervised hygienists: no less than 2 hygienists per 1 dentist (this limit does not apply to government employees).

DC requires dentist supervision. Board advises the Mayor on limiting the number of hygienists supervised by one dentist, with a minimum ratio of 2 hygienists to 1 dentist. This ratio limitation does not apply to government employees (DC or federal). Local anesthesia and nitrous oxide require direct supervision. DC is NOT a direct access jurisdiction.

DC is unique in several ways: the jurisprudence exam is a take-home written format (unusual among jurisdictions); CE requirements include mandatory LGBTQ cultural competency training (2 hours); three character references from persons who have known the applicant at least 1 year are required; and the dentist-to-hygienist ratio limitation has a specific carve-out for government employees. DC primarily accepts CDCA for the clinical exam, which is more restrictive than many states that accept multiple regional exams.
