Pass the National Board Dental Hygiene Examination (NBDHE). Format: 350 multiple-choice questions, computer-based. Passing score: 75.
Pass a clinical board examination. Alabama accepts: CDCA, CITA, CRDTS, SRTA, WREB.
Pass the Alabama dental jurisprudence exam. Alabama Jurisprudence Examination.
Obtain current CPR/BLS certification, pass a criminal background check, Infiltration Anesthesia Permit (optional, for local anesthesia — infiltration only, not nerve blocks), Oral Conscious Sedation Permit (optional).
Submit your application to the Board of Dental Examiners of Alabama. License fee: $150. Total initial fees: $150+.

Dental hygienists may perform prophylaxis, scaling and root planing, application of fluoride and sealants, radiographs, and periodontal assessments. Infiltration anesthesia requires a separate permit obtained through the board. Oral Conscious Sedation (OCS) permit also available.

Alabama is among the most restrictive states for dental hygienist practice - requiring direct supervision (dentist physically present) for all clinical procedures. Infiltration anesthesia (only infiltration, not block) was not authorized until 2018 and requires a separate permit. Nitrous oxide administration is not authorized for dental hygienists. The state also uniquely requires two character affidavits from licensed dental professionals for all applications.
Alabama is one of the more restrictive states for dental hygienists - requires direct supervision (dentist physically present) for all procedures.
