Your CDL is not a one-and-done credential — it must be renewed periodically, and your DOT medical certificate must be renewed every 2 years regardless of your CDL renewal cycle. If your medical certificate expires, your CDL will be downgraded to a non-commercial license until you submit a new medical certificate. Missing your renewal deadline can also result in having to retake tests.

The Medical Certificate: Your Most Frequent Renewal
Critical: DOT Medical Certificate
Renewal frequency: Every 2 years maximum (some drivers receive shorter certificates based on medical conditions)
What it is: Medical Examiner's Certificate (MEC), Form MCSA-5876
Who performs it: Must be conducted by a medical examiner listed on the FMCSA National Registry of Certified Medical Examiners (NRCME)
What happens if it expires: Your state will downgrade your CDL to a non-commercial driver's license. You cannot operate a CMV until you submit a new valid medical certificate.
Cost: Typically $80–$150 for the DOT physical (not covered by most health insurance)
Self-Certification Categories
When you apply for or renew a CDL, you must self-certify which operating category applies to you. This determines whether you need a medical certificate:
Interstate Non-Excepted: Operates in interstate commerce; must meet federal DOT medical requirements
Interstate Excepted: Operates in interstate commerce but is exempt from federal DOT medical requirements (e.g., certain federal/military/government drivers)
Intrastate Non-Excepted: Operates only in intrastate commerce; must meet state medical requirements

FMCSA Drug & Alcohol Clearinghouse
The FMCSA Drug & Alcohol Clearinghouse is a federal database that tracks drug and alcohol program violations for CDL holders. While it is not technically part of the CDL renewal process, it directly affects your ability to operate:
- Employers must query the Clearinghouse before hiring any CDL driver and at least once annually for current drivers
- Query fee: $1.25 per query per query
- Phase II (effective November 18, 2024): State DMVs must now revoke commercial driving privileges for drivers with a prohibited status in the Clearinghouse
- Drivers can register and view their own record at clearinghouse.fmcsa.dot.gov
What Happens If Your CDL Expires?
Medical certificate expires
Your CDL is downgraded to a regular (non-commercial) driver's license. You cannot operate any CMV. Once you provide a new valid medical certificate, your CDL privileges are typically restored without retesting.
CDL expires but within grace period
Most states offer a grace period for late renewal. You may be able to renew without retesting, though late fees may apply. Check your state's specific grace period — it varies.
CDL expired beyond grace period
You may need to retake the knowledge test, skills test, or both. Some states treat a long-expired CDL as a new application. ELDT requirements may also apply.
Special Renewal: HazMat Endorsement
The HazMat (H) endorsement has its own renewal requirements on top of regular CDL renewal:
- TSA security threat assessment: Must be renewed every 5 years ($85.25 fee). This is a federal requirement regardless of your state's CDL renewal cycle.
- Knowledge test: You must retake the HazMat knowledge test at each renewal in most states.
- Timing: Start your TSA renewal 90+ days before expiration to avoid a gap in endorsement status.
CDL Renewal Checklist
- Track both dates: Your CDL renewal date AND your medical certificate expiration date. They are independent.
- Schedule your DOT physical early: Don't wait until the last week. Some medical conditions require follow-up.
- Check for online renewal: Many states now allow online CDL renewal if you meet eligibility requirements.
- Update your Clearinghouse registration: Make sure your information is current.
- Budget for the medical exam: $80–$150 every 2 years, plus any specialist referrals required by the examiner.
- HazMat holders: Start TSA renewal 90+ days before expiration and budget for retesting.
