The Occupational Therapy Licensure Compact (OT Compact) allows OTAs to practice across member states with a compact privilege. Non-compact states may accept out-of-state credentials through endorsement or require a full application.
Iowa uniquely requires that the supervising OT directly participate in treatment at minimum every 12th visit, either in person or via telehealth. The state requires a separate $55 FBI background check fee in addition to the $120 application fee. Iowa combines OT and PT oversight under one joint Board of Physical and Occupational Therapy within the Department of Inspections, Appeals, and Licensing (DIAL). Out-of-state applicants have three pathways to demonstrate competency: recent CE hours, practice hours, or NBCOT exam pass.
Iowa requires 15 CE hours per 2-year cycle, with a minimum of 10 hours in clinical application of OT.
NBCOT proof of passing score must be sent directly to the Board office by NBCOT - copies not accepted.
Out-of-state applicants must show 15 CE hours, 2,080 practice hours, or recent NBCOT pass within 2 years.
