Paramedic programs are accredited by CoAEMSP (Committee on Accreditation of Educational Programs for the Emergency Medical Services Professions) through CAAHEP. Programs typically require EMT-Basic certification as a prerequisite and include extensive clinical rotations and field internship hours.

The NREMT Paramedic exam consists of a computer adaptive test (CAT) covering airway, breathing, cardiology, trauma, medical emergencies, and EMS operations. Most states require NREMT certification for initial licensure. Some states also require a psychomotor skills exam and/or a state-specific jurisprudence exam.

Paramedics operate under medical direction from a licensed physician. Online (direct) medical direction involves real-time communication with a physician, while offline (indirect) medical direction includes standing orders and protocols.
